Another employee, also suffered serious injuries.<\/span><\/p>\n<p style=\"background: white; box-sizing: border-box; font-variant-ligatures: normal; font-variant-caps: normal; orphans: 2; text-align: start; widows: 2; -webkit-text-stroke-width: 0px; text-decoration-style: initial; text-decoration-color: initial; word-spacing: 0px; margin: 0cm 0cm 7.5pt 0cm;\"><span style=\"font-family: 'Century Gothic',sans-serif; color: #333333;\">The Crown Court heard that on 18 November 2015, an electrical engineer, was working with a mechanical engineer in the basement of the site. They were working on an accumulator vessel when it exploded.<\/span><\/p>\n<p style=\"background: white; margin: 0cm 0cm 7.5pt 0cm;\"><span style=\"font-family: 'Century Gothic',sans-serif; color: #333333;\">A HSE investigation found that a flammable atmosphere developed within the accumulator as hydraulic lubrication oil was being drained from it. The flammable atmosphere was ignited by an electric heater within the accumulator.<\/span><\/p>\n<p style=\"background: white; box-sizing: border-box; font-variant-ligatures: normal; font-variant-caps: normal; orphans: 2; text-align: start; widows: 2; -webkit-text-stroke-width: 0px; text-decoration-style: initial; text-decoration-color: initial; word-spacing: 0px; margin: 0cm 0cm 7.5pt 0cm;\"><span style=\"font-family: 'Century Gothic',sans-serif; color: #333333;\">The investigation found that the company failed to assess the risks to which its employees were exposed when draining lubrication oil from the accumulator. Manually draining hydraulic lubrication oil from the accumulator by a procedure referred as a \u2018blow down\u2019 had developed through the Company employees\u2019 local custom and practice; this \u201cprocedure\u201d was not fully understood or consistently carried out by the Company\u2019s employees, exposing them to the risk of explosion.<\/span><\/p>\n<p style=\"background: white; box-sizing: border-box; font-variant-ligatures: normal; font-variant-caps: normal; orphans: 2; text-align: start; widows: 2; -webkit-text-stroke-width: 0px; text-decoration-style: initial; text-decoration-color: initial; word-spacing: 0px; margin: 0cm 0cm 7.5pt 0cm;\"><span style=\"font-family: 'Century Gothic',sans-serif; color: #333333;\">The Manufacturing company pleaded guilty to breaching Regulation 3 of the Management of Health and Safety at Work Regulations 1999, and have been fined \u00a31.8m and ordered to pay costs of \u00a3145,771.85.<\/span><\/p>\n<p style=\"background: white; box-sizing: border-box; font-variant-ligatures: normal; font-variant-caps: normal; orphans: 2; text-align: start; widows: 2; -webkit-text-stroke-width: 0px; text-decoration-style: initial; text-decoration-color: initial; word-spacing: 0px; margin: 0cm 0cm 7.5pt 0cm;\"><span style=\"font-family: 'Century Gothic',sans-serif; color: #333333;\">Speaking after the hearing, the HSE inspector said: \u201cThis incident, which had devastating consequences for all of those involved, was entirely preventable.
